HOW DO I ENROLL MYSELF AND MY DEPENDENTS?

• For new hires - you will be receiving an enrollment link from HR for you to enroll yourself and your dependents (if applicable)

• For old employee with new dependents - you will need to request from HR to send you a new link (hierarchy rules must be followed)

AFTER SUCCESSFUL ENROLLMENT, WHEN WOULD I RECEIVE THE HMO CARDS?

• You will receive your card/s 2-3 weeks after enrollment.

• However, while waiting for the cards, employee/s and/or dependent/s can still avail the HMO services by using the Maxicare Health Number provided by Maxicare and/or HR Team by presenting it to the clinics/hospitals.

HOW DO I RECEIVE MY MAXICARE HEALTH CARD NUMBER / CARD ID:

• Maxicare Health Number - you will receive an email from Maxicare

• Maxicare ID Cards - door to door delivery
