MATERNITY BENEFITS

1. How to file for a Maternity Benefit Application?
  • You must notify HR as soon as you find out you are pregnant. Early notification ensures your benefit is properly filed and processed on time.

2. Who can apply for Maternity Benefit?

You can apply through your employer if you are:

  • Currently employed, and

  • Have paid at least three (3) monthly SSS contributions within the 12 months before your childbirth or miscarriage.

3. What documents should I submit to HR to file my maternity notification?
  • SSS Maternity Notification Form (you can download this from the SSS website or fill it out through your My.SSS account)

  • Proof of pregnancy (ultrasound report or medical certificate indicating your expected delivery date)

  • Your SSS number

HR will then file your Maternity Notification to SSS through the Employer Portal.

4. What happens after HR files my maternity notification?

Once your notification is filed:

  • SSS will send a confirmation to your employer.

  • HR will provide you with a copy or acknowledgment of your notification for your records.

  • You may then proceed to plan your maternity leave schedule.

5. When will I receive my SSS maternity benefit?
  • Your employer will advance your SSS Maternity Benefit to you before or during your maternity leave.

  • The amount is based on your SSS salary credits and number of days of leave: 60 days for normal delivery or miscarriage 78 days for cesarean delivery

  • Additional 15 days if you are a solo parent

6. What should I submit after giving birth or miscarriage?

After delivery, please submit the following to HR:

  • Maternity Benefit Application Form (from SSS)

  • Child’s Birth Certificate (or Certificate of Fetal Death if applicable)

  • Two valid government IDs

  • Medical documents (if cesarean, miscarriage, or complicated delivery)

  • These documents will be used by your employer to claim reimbursement from SSS.

SICKNESS BENEFITS

1. How to file for a Sickness Benefit Application?

You must inform your employer within 5 calendar days after the start of your sickness or injury. If you fail to notify your employer within this period, you may lose your eligibility for the benefit.

2. Who can apply for Sickness Benefit?

You are eligible if you meet all the following conditions:

  • You were unable to work for at least four (4) consecutive days due to sickness or injury.

  • You have used up all your company sick leaves with pay for the year.

  • You have at least three (3) monthly SSS contributions within the 12-month period immediately before the semester of sickness or injury.

  • You have not received full pay from your employer for the same period of sickness.

  • You have notified your employer about your sickness or injury.

3. What documents should I submit to HR?
  • SSS Sickness Benefit Reimbursement Application (SBRA) – filled out and signed

  • Medical certificate or attending physician’s report indicating diagnosis and duration of illness

  • SSS Medical Report Form (if required for prolonged or complicated illness)

  • Hospital records, if confined

HR will review your documents and file your claim with SSS through the Employer Portal.

4. What if I am confined in a hospital?

You or your representative must notify your employer within 5 calendar days after discharge. If you are confined for more than 4 days, your SSS Sickness Benefit can cover your hospitalization period, subject to approval.

5. How much will I receive from SSS?

The sickness benefit is 90% of your Average Daily Salary Credit (ADSC).

Example: If your average daily salary is ₱1,000, you’ll receive ₱900 per day of approved sickness leave.

6. How many days of sickness benefit can I avail?
  • You can claim up to a maximum of 120 days per calendar year.

  • If your illness lasts longer than 240 days, it may be considered permanent disability, subject to separate SSS evaluation.

7. How is the benefit paid?
  • The employer advances the sickness benefit to you in your payroll.

  • The employer then applies for reimbursement from SSS.

  • You will receive the benefit through your company payroll once approved internally.

8. What if I fail to notify my employer on time?
  • If you fail to notify your employer within 5 days of your sickness, your employer’s reimbursement may be denied by SSS.

  • However, if you personally notify SSS within 5 days after recovery, you may still qualify for the benefit (if you file directly).
